== Mechanism of action == Tigecycline is a broad-spectrum antibiotic that acts as a protein synthesis inhibitor. These interactions are not observed in the binding of tetracycline. The findings indicate that tigecycline likely has a unique mechanism of action that prevents inhibition from ribosomal protection. It is a third-generation tetracycline derivative within a class called glycylcyclines which carry a N,N-dimethyglycylamido (DMG) moiety attached to the 9-position of tetracycline ring D. With structural modifications as a 9-DMG derivative of minocycline, tigecycline has been found to improve minimal inhibitory concentrations against Gram-negative and Gram-positive organisms, when compared to tetracyclines.

== Episode 7: Paralyzed == Joe is a 61 year old optimistic patient from Wallingford, CT. He was struck with a sudden and unexplained paralysis starting from his waist down, leaving him paraplegic. Ann is a 42 year old skeptic patient who is also from Wallingford, CT. She has intermittent paralysis where she could spend hours with the right side of her body immobile. She will then regain mobility but the paralysis always occurs again. Around the age of 59, Joe started to get pin & needles in his legs and at first he thought nothing of it until a year later he had numbness up to his knees that progressed to his waist. Eventually, the numbness turned into paralysis and Joe became a paraplegic at the age of 61. Shortly after becoming paralyzed, Joe went into a state of organ failure that doctors were able to get him out of but ever since he hasn't been able to leave the hospital. Despite visiting many hospitals such as Yale, Mayo, and Boston with no avail, he states he is still a firm believer in western medicine and is willing to try anything. Around the age of 40, Ann had a recurring pain in her chest that resembled heartburn and a month later noticed that the right side of her face looked off. When she brought this up to her primary care physician, he explained that she had full right side facial paralysis and needed to visit the emergency room as she might be having a stroke.

=== Selective serotonin reuptake inhibitors === Selective serotonin reuptake inhibitors (SSRIs), such as sertraline (Zoloft, Lustral), escitalopram (Lexapro, Cipralex), fluoxetine (Prozac), paroxetine (Seroxat), and citalopram, are the primary medications considered, due to their relatively mild side effects and broad effect on the symptoms of depression and anxiety, as well as reduced risk in overdose, compared to their older tricyclic alternatives. Those who do not respond to the first SSRI tried can be switched to another. If sexual dysfunction is present prior to the onset of depression, SSRIs should be avoided. Another popular option is to switch to the atypical antidepressant bupropion (Wellbutrin) or to add bupropion to the existing therapy; this strategy is possibly more effective. It is not uncommon for SSRIs to cause or worsen insomnia; the sedating noradrenergic and specific serotonergic antidepressant (NaSSA) antidepressant mirtazapine (Zispin, Remeron) can be used in such cases. For children and adolescents with moderate-to-severe depressive disorder, fluoxetine seems to be the best treatment (either with or without cognitive behavioural therapy) but more research is needed to be certain. Sertraline, escitalopram, duloxetine might also help in reducing symptoms.
